Permission to Use Image and Written/Verbal Statements

As an Attendee at Westfield Country Club (“WCC” herein), I understand that Ohio Farmers Insurance Company (“OFIC”), the owner of WCC, and OFIC representatives take photographs and videos at WCC: to market WCC to the public through various means, and to enhance communications with WCC members.

By signing this document below, I hereby grant to OFIC, the unrestricted right and license to reproduce, display, distribute, broadcast, transmit, create derivatives from, and otherwise use my likeness, voice, image, photograph(s), and personal statements, whether written or oral, in whole or in part, either alone or in combination with, or as a composite of, other matter, (hereafter “the Works”) for purposes of creating WCC promotional advertisements and marketing materials to the general public and within WCC.

I waive any right to inspect or approve any Works that may be so created and waive any claim with respect to the eventual use. The Works may be used at OFIC’s sole discretion, alone or in conjunction with any other material of any kind or nature, except that OFIC will not use the Works for any criminal or illegal purpose or in any manner inconsistent with community standards of decency. I understand and agree that OFIC is and shall be the exclusive owner of all right, title and interest, including copyright, in the Works.

I represent and warrant that I am at least 18 years of age and that I have read this consent form and fully understand its contents.
